California Consumer Privacy Act Request Form

California residents have the right to designate an authorized person or corporate entity to exercise rights granted to them under the California Consumer Privacy Act (CCPA). To make this designation, California residents may complete and submit
this form. To the extent we are unable to verify the identity of the person submitting this form, we may request additional information from the person making the submission. Authorized agents that have been provided a power of attorney from a
requester may submit requests directly.

Meridian Servicing will not discriminate against any consumer for exercising their rights under the CCPA or the California
Privacy Rights Act (CPRA).

Submission of a privacy request does not affect any underlying account obligations. Requests to
delete are subject to several exceptions under the law.

Meridian Servicing will confirm receipt of this request within 10 business days and will respond within 45 calendar days,
